A twenty-eight-year-old woman is seeking a host home in Fremont County. She is currently living in Chaffee County, but would like to move to a host home in Fremont County. This individual would like to continue to learn skills such as cooking, budgeting, scheduling appointments, and more as she works towards living independently. She has learned several new skills over the past couple of years. She would like to get a job working in her community and make new friends. This person is very independent, but needs support with medication management, budgeting, etc. She takes pride in her appearance, enjoys spending time with friends, fishing, going out to eat, as well as arts and crafts. She is a very talented artist. She loves horses and other animals. She also likes sports and she’s an avid fan of the Colorado Rockies and the Denver Broncos. She enjoys walking animals and playing with them. She likes to keep busy and enjoys helping others. This individual takes good care of her belongings and keeps her bedroom tidy. The host home contractor’s responsibilities include providing supervision and supporting this individual with medications, money management, shopping, etc. Host Home Contractors are responsible for basic home care and related responsibilities. This includes providing nutritious meals, recreational activities, ensuring access to community activities, and supervision based on the needs of the individual.

Host Homes offer an alternative to a group home setting for our adults and offer you the opportunity to be a contract provider of services. A Host Home is a community residential setting that consists of contracting with an individual(s) to provide the residential care needed for a person receiving services who is unable to live independently.
