Case Management Transition Open House

There are exciting changes taking place at Starpoint and we want you to help answer any questions that you may have! We will be hosting a number of in-person and virtual open houses to help you learn about how these exciting changes will enhance your services and impact our community.
The State of Colorado is transitioning to Conflict Free Case Management within rural communities, and the transition within Starpoint will take place on November 1, 2023. Starpoint is excited to implement Conflict Free Case Management requirements from the Centers for Medicaid and looks forward to continuing to provide high-quality services in our rural communities.

In person options are come and go at 517 N Diamond Ave in Canon City. They include the following dates:
– October 16th from 2:00 PM to 3:30 PM
– October 16th from 5:00 PM to 6:30 PM
– October 17th from 9:00 AM to 11:00 AM
– October 23rd from 2:00 PM to 3:30 PM
– October 23rd from 5:00 PM to 6:30 PM
Zoom options are available on the following dates:
– October 17th from 4:30PM to 6:30PM
– October 18th from 4:30PM to 6:30PM
– October 19th from 4:30PM to 6:30PM
